Weight Management – thoughtful, clinical support for every body

We believe that conversations about weight should happen only if and when you want them to. Healthcare should be kind, respectful, and customized to you.

Care that treats the whole you—not just the number on the scale

Our weight management program is based on science, not shame. We know that weight is complex and it’s shaped by your biology, health history, medications, sleep, stress, and more. Not just what you eat or how much you move. Medical weight management needs to be overseen by a clinician because your health and safety come first. Prescription medications for weight must be carefully chosen, started, and adjusted based on how your body responds.

We take a whole-person approach by listening, looking at your medical history, and talking with you about your health goals and any challenges you're facing. Then, we build a plan together that supports both your body and your life.

knownwell works closely with your existing primary care provider to coordinate your care. If you don’t have a primary care provider, you can establish care with a knownwell clinician.

Meet Susan

Susan is 58 and recently retired. With her first grandchild on the way, she’s excited to stay active and enjoy this next chapter—traveling, connecting with family, and focusing on her well-being.

She’s navigating menopause and has tried several diet trends in the past that didn’t give her the lasting results she hoped for. Now, Susan is looking for a care team that truly listens and works with her to build a sustainable plan that fits her life.

Getting started at knownwell

Select moments along the timeline to explore Susan’s journey.

Day 1

Susan is referred to knownwell by her primary care physician

She registers and books her first visit with a knownwell clinician. With her consent, we gather her medical records and have her fill out a quick questionnaire so that she’s known well— even before her first visit.

Day 7

Susan attends her first visit with a licensed physician

Susan’s glad the visit is 2-3x longer than the average clinic. She and Dr. Fitch build a comprehensive care plan together.

She’s excited for her personalized plan, including medical nutrition counseling, guidance on sleep and stress, and an FDA approved GLP-1 medication.

Day 8

Coverage support team

knownwell’s prior authorization team works to get Susan’s GLP-1 medication covered by insurance and keeps her in the loop.

Day 12

Clinical Dietitian support

Susan has her first visit with Arianna and works with her to create a nutrition plan that is realistic and practical.

Day 14

Virtual medication support

Susan is ready for her first dose of GLP-1 and is a bit nervous about the injection process. She meets virtually with Mona, a pharmacist at knownwell, who walks her through it.

Ongoing personalized care support

Day 30

Ongoing support

Susan starts implementing lifestyle changes while continuing medication and keeping track of her progress and messaging with her care team as needed in the patient app.

Day 60
Patient

Ongoing lifestyle support

Susan optionally checks in with her care team, including Deb, a Nurse Practitioner, on how her lifestyle changes are going and to help coach her on sleep, stress, and other potential impediments to her goals.

Day 180
Patient

6 Month visit

Susan has her 6 month check in visit and is happy with her progress. She’s lost 40lbs! Dr. Fitch also helps adjust her blood pressure medication, given she now needs a lower dosage.

Day 212

Follow-up visit

Susan is worried she’s losing momentum and has hit a plateau, so she schedules a follow-up visit.

Susan and Dr. Fitch implement an intervention plan to course correct, she redoubles her lifestyle changes and goes on a higher dose of her GLP-1 medication.

Expert support for every step of your journey

Making lifestyle changes that stick takes more than willpower—it takes support. At knownwell, our care is built to be sustainable, not overwhelming. You’ll work with a dedicated team of Physicians, Advanced Practice Clinicians, Clinical Dietitians, Nurses, and Pharmacists who help you develop a personalized approach to eating that fits your life.

Most of our physicians are Board-certified in Obesity Medicine (ABOM), bringing advanced training and expertise to your care. Our nurse practitioners and physician assistants are specially trained in weight management care. They understand the complex science of weight and metabolism and offer evidence-based tools that go beyond one-size-fits-all advice. Whether that includes the latest weight loss medications or movement and nutrition programs, knownwell clinicians work with you to create a plan that feels right and works for you.

We’re here for the long term. That includes helping you break through plateaus, navigate weight regain, or course correct in a way that aligns with your goals and values. Whatever stage you’re in – just getting started, making progress, or getting back on track -  we meet you there with compassion and expertise.

At every step, our clinical team takes the time to listen, understand what matters to you, and make care decisions together. Because you deserve care that’s collaborative, compassionate, and rooted in dignity and a team that never stops believing in your capacity to grow and thrive.

Centers that are comfortable, private, and welcoming

knownwell centers are designed to feel significantly different from traditional healthcare settings. From the moment you walk in, you’ll notice the difference: warm lighting, calming colors, and welcoming spaces that don’t feel clinical. We’ve prioritized comfort, dignity, and psychological safety in every detail from wide hallways and size-inclusive seating to quiet, calming reception areas.

You’ll never be weighed in a hallway, and we only ask for your weight when it’s relevant to your care plan, always with your consent and in a private setting. Our exam and consult rooms are spacious and calming and have enhanced sound privacy for your confidentiality and designed for real conversations and personalized care. More than just beautiful, our spaces are built to support trust and connection because where you receive care matters just as much as how you receive it.
